How can claims be submitted?

• Insured must submit duly completed claim form and any other required documents as may be required by Singlife within 30 days of the date of Accident.
• Claims forms and supporting documents should be sent to managed_care3@singlife.com. Please visit www.singlife.com to find out more about Claims Process.
